Members from the WOW Total Health/Wealth Solution Cashback Campaign are automatically enrolled in the challenge and can skip Step 1.

The AIA Vitality Health Check is FREE at participating Guardian outlets nationwide! Prior appointment is required.

HOW DOES THIS WORK?

Earn 10,000, 20,000 or 30,000 points to hit Silver, Gold and Platinum status respectively. Get rewarded each time you level up within your membership year.

FREQUENTLY ASKED QUESTIONS

Q. Who is eligible for the Level Up Rewards?
A. AIA Vitality members who successfully submitted the challenge form and completed the AIA Vitality Health Check prior to moving up to a new status will be eligible.

Q. Can I fill out the challenge form first before I move up my status?
A. Yes! We encourage members to submit the form as early as possible from 1 January 2022 onwards as only new statuses achieved after form submission will be eligible for rewards.

Q. How will I get my cash reward after I have levelled up my status?
A. Upon levelling up your status, your reward will be emailed to the email address you indicated in the form within 1-4 weeks.

Q. What is the duration of this campaign?
A. This campaign begins from 1 January 2022 until further notice. AIA Vitality reserves the right to amend the campaign period at any time.

Q. I have earned Level Up Rewards back in 2021. Do I still qualify?
A. Yes, new statuses achieved from 1 January 2022 onwards are eligible for Level Up Rewards. You will need to submit the challenge form in 2022 and complete your AIA Vitality Health Check within your membership year to be eligible.

Q. I have completed my AIA Vitality Health Check before 1 January 2022, but it is still within my current membership year. Have I unlocked the reward?
A. Yes, as long as you have completed this step within your current membership year. Just make sure to submit the challenge form to join the challenge!

Q. I have completed 1 to 3 tests out of 4 from the AIA Vitality Health Check. Does this mean I have unlocked my Level Up Reward?
A. No, you will need to submit your results for all 4 tests (BMI, blood pressure, blood glucose, cholesterol). We evaluate eligibility based on the submission date of your test results. You may complete this step at any time to unlock your reward, but only new status achieved after completion will be eligible for the reward.

TERMS & CONDITIONS

1. Valid only for new Vitality status achieved from 1 January 2022.
2. To qualify for Level Up Rewards, members must have successfully filled out the challenge form, submitted all 4 test results under the AIA Vitality Health Check on My AIA app within the current membership year before achieving the next status level.
3. Members from the WOW Total Health/Wealth Solution Cashback Campaign are automatically enrolled in the challenge and don’t need to resubmit the challenge form.
4. Only members with active AIA Vitality memberships at time of reward fulfilment will receive their reward.
5. Campaign period begins on 1 January 2022 until further notice. AIA Vitality reserves the right to amend the campaign period at any time.
