1) You must choose one of the four core benefits below, with minimum sum assured of RM1,000. You may then choose to add any of the other 25 optional benefits.

- Accidental Death and Dismemberment Benefit
- Accidental Death Benefit
- Accidental Dismemberment Benefit
- Accidental Death Benefit + Accidental Dismemberment Benefit (different sum insured)

2) The 5 covered Infectious Diseases are Japanese Encephalitis, Malaria, Measles, Tuberculosis and Hand, Foot and Mouth Disease.
3) Benefits related to Infectious Diseases and Dengue Fever will commence after fourteen (14) days from the effective start date of the Period of Insurance.
4) Each policy can only select either one of Daily Hospital Income due to Infectious Diseases OR Hospital Income due to Infectious Diseases between the two benefits.
5) Each policy can only select either one of Daily Hospital Income due to Dengue Fever OR Hospital Income due to Dengue Fever between the two benefits.
6) Each policy can only select either one of Accidental Weekly Indemnity Benefit OR Daily Recuperation Allowance between the two benefits.
7) You must give us written notice of a claim within 30 days of the accident that causes injury. If death occurs, an immediate notice of claim must be given to us.
